New Clients
Please fill out all required forms before your arrival. All forms are sent via email from Madison Grace Luxury Aesthetics prior to appointments. If you are unable to complete the paperwork before your visit, please arrive 15 minutes early to allow extra time for check-in.
Clients’ Right To Know
We strive to exceed our clients’ needs. We believe that suggesting products best suited for them is as essential as the service we perform. Therefore, our client’s right to know policy advises each client on the products and techniques best suited to maximize the benefits of their service.
Refund and Return Policy
No refunds are made for products, service packages and pre-paid treatments once they are purchased. If for some reason you are not able to use an un-rendered, pre-paid service, you may do a one time exchange of the unused portion toward other services. Unopened products may be exchanged for alternative products within 7 days of purchase. Products from the medical weight loss program may not be returned or exchanged once they have been purchased.
